How to use it

The Implementation Alignment Diagnostic™ is a 16-question self-assessment. Use it to reflect on the conditions affecting an initiative, strategic plan, organizational change, or CHNA implementation strategy.

Your responses provide a starting point for understanding where implementation may need attention.

Four dimensions of the implementation environment
