Implementation Architecture™ is the design of the organizational conditions that turn strategy into repeatable results.

A strategic plan can identify the right priorities. Implementation asks a different set of questions: Who owns each decision? How does work move across teams? What gets funded first? When will leaders see progress? What changes when the evidence says an approach is not working?

HSP works with leaders to answer those questions and put the answers into practice. The result is a clear way to coordinate decisions, delivery, measurement, and learning around the work that matters.

When the CHNA is complete, implementation begins.

A Community Health Needs Assessment identifies priorities. Communities see their value when those priorities shape decisions, direct resources, strengthen partnerships, and produce measurable progress.

HSP applies Implementation Architecture to CHNA follow-through: who owns the work across the hospital and community, how priorities are sequenced, how partners participate, what progress is measured, and how leaders learn and report over time.
